Stock Certificate

If you are donating securities for which you have the certificate in your name, please do the following:

1. Do not sign the certificate.
2. Sign a separate stock power and have your signature medallion guaranteed at your broker’s or your local bank. (Call your bank first. Not all banks can accommodate you.)
3. Hand-deliver or mail, using certified mail with return receipt or an overnight service, the stock certificate and the stock power.
